• Resource Coordination

    The main goal of Resource Coordination is to meet patients' needs and preferences in the delivery of high-quality, high-value health care. This means that the patient's needs and preferences are known and communicated at the right time to the right people, and that this information is used to guide the delivery of safe, appropriate, and effective care.

  • Do I Qualify for Resource Coordination?

    The patient must be essentially homebound, meaning trips away from the home must be infrequent or of short duration. Medicare does allow trips outside of the home for religious services and medical treatment. Care must be intermittent (part time), medically necessary, and ordered/approved by your physician. The patient must have a skilled need.

    Resource Coordination Advantages

    • Complete assessment of patient needs and assistance with obtaining services to meet those needs
    • We assist families with transitioning from facility to home with resources to ensure all health and safety needs are met.
    • SHH will coordinate with hospital CM patients' discharge planning to ensure transitioning needs are met reducing re-hospitalization.
    • We provide help with any applications for housing, medicaid, medicare, disability, etc.
